Sign-On Bonus for Full-time $2,500

Cornerstone Rehab is seeking an experienced and motivated Senior Physical Therapist to oversee clinical responsibilities across multiple facilities.

Responsibilities:

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with patients’ needs and capabilities.
• Travel to nearby centers as needed

Qualifications:

• Valid State Physical Therapist License and Credentials
• SNF and/or Acute care experience
